To obtain a medication refill please contact your

pharmacy and have them initiate a refill request

to our office via our fax which is 954-567-2490.

This policy will guarantee that your prescription will

go to the correct pharmacy of your choice. 

It eliminates duplicate prescriptions and

will confirm that you are receiving the

correct dosage and quantity.

For a new medication prescription you will need to

make an appointment with the doctor.
